  • Conducts/coordinates healthscreens/medical surveillance

  • Orders lab values

pertinent to the employee at the time of the health screen.

  • Completes assessment

and reviews health history of employee at the time of


  • Coordinates

appropriate plan of care/referrals based on relevant findings.

  • Administers TB testing/T-Spot testingto all employees according to state regulation and hospital policy.

  • Distributes

individual written notices to all employees at least two (2)

weeks prior to the due date.

  • Provides written

instructions for completion of testing.

  • Educates employees on

specific requirements and follow-up as


  • Refers to Public

Health Department/primary care physician and obtains

chest x-ray upon significant reaction to PPD according to Kentucky

Administrative Regulations.

  • Coordinates the review and follow-up ofall incident reports received in the Health Office.

  • Inspects all incident

reports within 24 hours of arrival in the Health Office

and forwards copy to WC Coordinator.

  • Completes

investigation on reports requiring follow-up within 48 hours of


  • Draws blood on

healthcare worker involved in exposure and completes

HIV counseling.

  • Sends written opinion

to healthcare worker regarding status of

exposure and information obtained on source patient.

  • Coordinates follow-up

testing/treatments in accordance with Employee

Health protocols (i.e., TB test, HIV test, etc.).

  • Draws blood for

follow-up testing per exposure protocol and employee


  • Provides immunization program toemployees per Employee Health policy.

  • Provides hepatitis-B

and education on a continual basis.

  • Administers standard

flu vaccine during fall months.

  • Administers

tetanus/diphtheria vaccine to employees as indicated

following an injury every 10 years as recommended by Public Health

Service Advisory Committee on Immunization Practices. Provides 1 time

dose Adacel as indicated per established guidelines.

  • Provides education on

MMR and administers vaccine according to

established guidelines. Maintains records on immunization status


employee health file.

  • Provides education on

varicella vaccine (chicken pox) and administers

vaccine according to established guidelines.

  • Provides other

immunizations as indicated.

  • Maintains information

on employee’s immunization status.

  • Evaluates employees in need of healthservices and arranges for appropriate treatment or referral.

  • Provides first aid

for employee injuries.

  • Provides palliative

care for non-occupational illness and injuries and

refers as indicated.

  • Counsels employees on

health concerns and illness upon request.

  • Reviews health

records annually to assure current information is


  • Maintains accurate

and up-to-date records as indicated by prompt

retrieval of desired information.

  • Collaborates on activities to promoteand/or maintain employee health.

  • Attends meetings when

required and participates on committees (i.e.,

Wellness, Infection Control Liaison, and subcommittees).

  • Reviews the Employee

Health Manual when indicated or bi-annually and

collaborates on additions to maintain accurate and current information.

  • Participates in

health screening events of KentuckyOne Health

requested to provide information to the public and promote a positive


Coordinates andadministers drug tests according to KentuckyOne Health policy.

  • Completes drug test

on prospective employees as soon as possible upon

notification employment


  • Coordinates employee for-cause drug test upon

direction of Human


  • Coordinates and completes random drug tests on

employees according

to the schedule indicated by Human Resources.

  • Notifies employment personnel of drug test

confirmation results upon

receipt of results.

  • Maintains confidentiality ofinformation of the Human Resources Department.

  • Maintains confidentiality of departmental

information according to

established procedures with no reported errors.

  • Releases confidential information only in

accordance with the

confidentiality policy and/or approval of the director of Human


  • Contributes to the efficient operationof the Human Resources Department.

  • Completes special projects as assigned by the

Manager according to

established time frames.

  • Prepares and maintains reports of results as

requested by the Manager.

  • Identifies and reports equipment problems

which require maintenance

or repair to Supervisor/Manager.

  • Facilitates the efficient operation of the

department by answering the

telephone assisting callers or visitors in any way possible.

  • Performs other duties as assigned.

  • Completes mandatory

education and in-service requirements with the

review year(s), as established by departmental policy and state nursing

practice policy.

  • Attends and actively participates in

departmental meetings.

  • Participates in self-evaluation by identifying

strength/limitations, and establishes and meets professional individual

goals to assist the department in achieving its established


  • Participates in

educational programs related to practice area and keeps

abreast on current updated literature as demonstrated by

applying/contributing this knowledge in appropriate staff meetings and

daily clinical practice.

  • Demonstrates an

  • Maintains patient

  • Associate Degree in Nursing;Bachelors preferred (may be in other health/safety related field)

  • Minimum of 2 years of nursingexperience; 1 years Employee Health experience.Familiar with Worker’s Compensationlaws; experience with OSHA, Joint Commission and OIG.
